Noise nuisance report

You should use this form to tell us about a noise problem.

You will need to provide some information including:

• the address or location of the noise
• type of noise (such as, loud music, a barking dog or an extractor fan)
• when and for how long the noise normally occurs
• the way the nuisance affects you (such as, it prevents you from sleeping) 

We'll also ask for your:

• name
• phone number
• email address

You can use this from to send us any supporting evidence such as video files or sound recordings.
